Do you look after, help or support a relative, friend or neighbour on a regular basis?
This could be because they have a long or short term severe
medical condition, or have a sensory or physical disability or are older and frail.
It could be your wife, husband, aunt, uncle, father, mother, niece, nephew, sister, brother or a neighbour or friend, who relies on you to do the extra things they cannot do.
Sometimes very small but important every day tasks, sometimes more personal or financial support.
They could be living with you or living in their own home or in a residential or nursing home.
Please let your GP know so he/she can make sure you have the right support at the right time.
It would be helpful if you request and complete a Carers Registration form (pdf link above) and hand it in to reception as soon as possible.
Even if the person you care for does not want to share their own information you can register as a carer so you can be signposted to the right support for yourself.
